We ask that our clients peruse the form below. We suggest you fill out our intake form prior to your appointment so that your time spent in office will be focused on your current concern. In addition, please review the Informed Consent document, as this provides you with information regarding our confidentiality agreement. This form will be reviewed with each client at their first session to ensure his/her understanding and agreement, prior to the start of any therapy.
